Transcript Requests
Please contact Rhonda Galis, Registrar at West Campus, at 630.295.5340 for information regarding our new electronic transcript system if you need transcripts sent to a college or university. For transcripts needed for employment purposes, please download the request form by clicking here. Request must be mailed to the Registrar, Lake Park High School West Campus, 500 W. Bryn Mawr Ave. , Roselle , IL 60172 . Faxes or email will not be accepted. Please include $1.00 per transcript and allow a turn-around time of 2 – 3 weeks.
